What to Expect: Insurance, Payment & Billing Information
As a new patient, you will need to complete a new patient information form along with verifying your current insurance. Please bring a copy of all insurance cards and a photo ID. We will need to make photo copies and place in your chart. If you have more than one insurance, please advise which should be primary in order to avoid any delays in processing claims. If you have a HMO or PPO plan, please verify Illinois CancerCare is in network with your insurance. Also, if you have any changes in coverage, please contact the Business Office right away. We will need to verify is pre-authorization or pre-certification is needed prior to any upcoming appointments.
Please be prepared to fill out information and questionnaires involving personal information, medications, and possibly family history. At this time, you will be presented with all standard new patient agreements for billing purposes and release of information.
Co-payment is expected at time of service. We accept checks, Visa, Mastercard, or Discover. Sorry, no cash is accepted.
Responsibility for all bills incurred remains with the patient. You will be billed for any balance due after we have heard from your insurance carrier.
